Alcohol, Food and Erection Medication: What Changes
The short answer
A high-fat meal slows sildenafil by about an hour and lowers its peak by about 29 percent, and it cuts vardenafil's peak by 18 to 50 percent; tadalafil is unaffected by food. A drink or two did not worsen the blood pressure drop from these medicines in label studies, but heavy drinking (about five or more drinks) with tadalafil caused dizziness and low standing blood pressure, and alcohol works against erections on its own. Grapefruit juice blocks the enzyme that clears all three, so it can raise drug levels.
Date night has a menu, and it usually runs steak, a couple of drinks, then the main event. Sound plan. The catch is that two of those courses can quietly change how the third one goes, and the pill you took at the table is caught in the middle.
None of this means eating a salad in the dark. It means knowing which medicine you take, how food and alcohol treat it, and where the real line sits. Most of it comes down to timing. The rest comes down to not overdoing the drinks, which was true long before anyone invented a pill.
We all need a little help getting hung sometimes. Dinner does not have to get in the way of it.

Does food change how erection medication works?
Yes for sildenafil and vardenafil, no for tadalafil. The sildenafil label reports that a high-fat meal slows absorption, delaying the peak by a mean of 60 minutes and lowering it by 29 percent 1. MedlinePlus puts it plainly: take it with a high-fat meal and it will take longer to start working 4.
Vardenafil has the same weakness. Two food-effect studies found high-fat meals reduced its peak concentration by 18 to 50 percent, although the label still says it may be taken with or without food 3. Mayo Clinic notes the body takes longer to absorb both sildenafil and vardenafil after a high-fat meal or alcohol 6.
Tadalafil is the exception. Its label says it may be taken without regard to food, and no meal effect is described 2.

Can you drink alcohol with sildenafil, tadalafil or vardenafil?
A drink or two is generally compatible; a night of heavy drinking is where the trouble starts. In the sildenafil label’s interaction study, 50 mg given with alcohol at 0.5 g/kg, enough to reach a mean peak blood alcohol of 0.08 percent, did not potentiate alcohol’s blood pressure lowering in healthy volunteers 1. Vardenafil 20 mg with the same alcohol dose, about 40 mL of pure alcohol, gave the same result 3.
Tadalafil’s label is the most explicit. It reminds patients that both alcohol and tadalafil are mild vasodilators, so their blood pressure lowering can add up, and that substantial drinking, about 5 units or more, can raise heart rate, drop standing blood pressure and cause dizziness and headache 2. In the study behind that warning, more men on tadalafil plus alcohol at 0.7 g/kg had clinically significant blood pressure drops, some reported postural dizziness, and orthostatic hypotension was observed; at 0.6 g/kg, none of that happened 2. MedlinePlus translates the line as more than five glasses of wine or five shots 5.

Why does alcohol make erections harder in the first place?
Because the medicine can only amplify a signal that is already there, and heavy drinking tends to turn that signal down. NIDDK lists drinking too much alcohol among the health-related causes of erectile dysfunction, alongside smoking, inactivity and recreational drugs 7. Alcohol is also a mild vasodilator, which is the tadalafil label’s reason for warning about the combined blood pressure effect 2. Mayo Clinic adds a practical wrinkle: alcohol slows the absorption of sildenafil and vardenafil, so the pill arrives later on top of everything else 6.
The pills do not create arousal. Every label says they do nothing without sexual stimulation, so a pill taken after too many rounds is fighting the alcohol rather than helping you 123.
What about grapefruit juice?
Grapefruit juice can raise levels of all three medicines, and the labels for two of them say so directly. FDA explains the mechanism: grapefruit juice blocks the CYP3A4 enzyme in the small intestine, so more of a drug enters the blood and stays in the body longer 8. Sildenafil, tadalafil and vardenafil are all cleared mainly by CYP3A4 123.
The tadalafil and vardenafil labels state that grapefruit juice would likely increase exposure to the drug 23. The sildenafil label does not mention grapefruit by name, but it advises considering a 25 mg starting dose in men taking strong CYP3A4 inhibitors such as ketoconazole, itraconazole or erythromycin 1. MedlinePlus tells users of all three to talk with their doctor about grapefruit 45.
Higher drug levels mean more of the dose-related side effects: headache, flushing, indigestion and, with sildenafil, the color-tinged vision that showed up in 11 percent of men at 100 mg 1. Nobody needs a citrus ban; just do not make grapefruit juice the chaser.
How should you time dinner, drinks and the pill?
Match the plan to the medicine. Sildenafil can be taken 30 minutes to 4 hours before sex, so taking it before the meal, or with a lighter one, keeps its timing predictable 14. Tadalafil frees you from the menu entirely, whether you take it as needed or as a low daily dose 2. Its one rule is the drinks: stay under the 5-unit line and remember that the medicine is still on board the next day 25. Whichever you take, never combine it with nitrates or poppers, and never stack two erection medicines 123.
When to talk to a clinician
If you feel lightheaded or faint when you stand up after mixing alcohol with any of these medicines, sit or lie down, skip the drive, and call if it does not pass. Get emergency care for chest pain during sex, an erection lasting more than 4 hours, or sudden loss of vision or hearing 123. Never take these medicines with nitroglycerin, isosorbide or any nitrate, or with poppers 123.
Tell your clinician honestly how much you drink; heavy drinking is a cause of erectile dysfunction worth treating in its own right 7. Also mention blood pressure medicines, alpha blockers, antifungals, HIV medicines and some antibiotics, since those change the safe starting dose 123.

Heavy drinking is a different story: it lowers blood pressure, can leave you dizzy, and works against erections in its own right.
