How Long Does Sildenafil Take to Work? Timing That Matters
The short answer
Sildenafil usually starts working within 30 to 60 minutes, and the label says to take it about 1 hour before sex, anywhere from 30 minutes to 4 hours ahead. Its effects typically last about 4 to 5 hours, but a high-fat meal can delay the start by about an hour, and it only works if you are sexually aroused. Take it no more than once a day at the dose your clinician prescribes.
A cowboy does not saddle the horse after the cattle have already left. Timing is the whole job, and the same goes for sildenafil. Most of the “it didn’t work” stories we hear are really “I took it at the wrong time, on top of a bacon cheeseburger, and then waited for something to happen while watching the game.”
Sildenafil is a good medicine with a specific rhythm. Learn the rhythm and it is remarkably dependable. Ignore it and you will spend an evening blaming a pill for a scheduling problem.

How long does sildenafil take to work?
For most men, 30 to 60 minutes. The label recommends 50 mg taken about 1 hour before sexual activity and notes it may be taken anywhere from 30 minutes to 4 hours before 1. MedlinePlus repeats that window and calls about 1 hour “the best time” 3. Mayo Clinic adds that it works best on an empty stomach, an hour before sex 4.
Sildenafil is absorbed quickly, then cleared quickly. Both the drug and its main breakdown product have half-lives of about 4 hours 2. That is why a dose taken at dinner has largely left by breakfast, and why the medicine does not carry over to the next night.
Why does a heavy meal slow sildenafil down?
Fat slows the stomach, and a slower stomach means slower absorption. When sildenafil is taken with a high-fat meal, the rate of absorption drops, the time to peak concentration is delayed by about 60 minutes, and the peak level falls by about 29 percent 2. MedlinePlus puts it in plain words: with a high-fat meal, it will take longer for the medication to start to work 3.

How long does sildenafil last?
About 4 to 5 hours for most men, according to Mayo Clinic, which fits a half-life of roughly 4 hours 24. The label’s dosing window of up to 4 hours before sex says the same thing from the other direction 1.
“Lasts” needs a definition. Over those hours sildenafil keeps the plumbing responsive rather than producing one continuous erection: an erection comes more easily when you are aroused, and returns more readily after orgasm if the two of you are not finished. Without arousal, the label says, the drug has no effect 2.
What dose of sildenafil should you take?
The usual starting dose is 50 mg, adjusted up to 100 mg or down to 25 mg based on how well it works and how well you tolerate it 1. Side effects climb with dose: in the fixed-dose trials, headache was reported by 16 percent of men on 25 mg, 21 percent on 50 mg and 28 percent on 100 mg, versus 7 percent on placebo, and vision changes rose from 1 percent to 11 percent at 100 mg 2.
Some men need to start lower. The label says men on alpha-blockers for prostate or blood pressure should be stable on that medicine first and start sildenafil at 25 mg, and men taking ritonavir should not exceed 25 mg in 48 hours 1. What should you do if sildenafil does not work?
Do not take another pill, and do not give up after one try. The American Urological Association guideline tells clinicians to explain that sexual stimulation is necessary and that more than one trial with the medication may be required to establish efficacy, then to titrate the dose 5.
The evidence behind that advice is humbling. In a study of 100 men referred as sildenafil non-responders, 56 had used it incorrectly: 45 had never tried the 100 mg dose, 32 had taken it on a full stomach right after a meal, 22 had taken it just before starting sex, and 12 did not know arousal was required 6. Most had made fewer than four attempts 6. After proper instruction and dose adjustment, roughly a third or more of men who had failed a PDE5 inhibitor responded 6.
So the checklist is: light meal, about an hour ahead, real arousal, four or more honest attempts at a dose your clinician has approved. If it still falls short, that is useful information, not a verdict. Low testosterone, uncontrolled blood sugar, anxiety and certain medicines can all blunt the response, and each has its own fix 56.
When to talk to a clinician
Seek emergency care for an erection lasting more than 4 hours, sudden loss of vision in one or both eyes, sudden hearing loss or ringing, chest pain during sex, or fainting 13. Never take sildenafil if you use nitroglycerin, isosorbide or any other nitrate, poppers (amyl or butyl nitrite), or riociguat; the combination can crash your blood pressure 13.
Message your clinician if you get headaches, flushing or indigestion that bother you, if you take alpha-blockers, blood pressure medicine, antifungals or HIV medicines, or if the medicine has not worked after several well-timed tries 15.

How long does sildenafil stay in your system?
The half-life is about 4 hours, meaning half the dose is cleared every 4 hours. Effects often last 4 to 5 hours, and by 24 hours the blood level is a tiny fraction of the peak. Nitrates are still off limits during that whole stretch.
Can I take sildenafil twice in one day if the first dose did not work?
No. The maximum recommended frequency is once per day, and a second dose raises the risk of headache, flushing, vision changes and a blood pressure drop without fixing the reason the first one fell short. Talk with your clinician about the dose and timing instead.
Does sildenafil work without arousal?
No. The label states that sildenafil at recommended doses has no effect in the absence of sexual stimulation. It helps blood flow into the penis when you are aroused; it does not start an erection on its own or increase desire.
Will sildenafil work the first time?
Often, but not always. The American Urological Association guideline says men should be told that more than one trial may be needed to establish whether it works, and a study of 100 men labeled non-responders found most had made fewer than four attempts, often on a full stomach or at too low a dose.
