Vardenafil Explained: The Option Most Men Have Not Heard Of

The short answer

Vardenafil is a PDE5 inhibitor for erectile dysfunction in the same family as sildenafil and tadalafil. It is taken about 60 minutes before sex at 5 to 20 mg (usually starting at 10 mg), works for roughly 4 to 5 hours, and carries the same nitrate ban, plus a specific caution for men with long QT syndrome or on certain heart-rhythm drugs. It is available as an inexpensive generic tablet and as a dissolving tablet.

Every rodeo has a headliner, a crowd favorite, and a third rider nobody bet on who quietly puts up a good score. Vardenafil is the third rider. Sildenafil got the fame, tadalafil got the 36-hour legend, and vardenafil got a spot on the pharmacy shelf and very little conversation.

That is a shame, because it is a solid medicine with a couple of quirks worth knowing. It works about as fast and as long as sildenafil, has its own side-effect fingerprint, and carries one heart-rhythm caution the others do not.

We all need a little help getting hung sometimes. Occasionally the help you have not heard of is the one that fits.

Key facts

  • Vardenafil is taken about 60 minutes before sex, starting at 10 mg and adjusted between 5 and 20 mg, no more than once a day 1.
  • Its half-life is 4 to 5 hours, and Mayo Clinic says it usually works for 4 to 5 hours 13.
  • It can be taken with or without food, but a high-fat meal cut peak levels by 18 to 50 percent in studies 1.
  • Men with congenital long QT syndrome or on Class IA or III antiarrhythmics should avoid it 1.
  • Nitrates, poppers and riociguat are absolutely off limits, as with every PDE5 inhibitor 12.
  • It is available as a generic tablet and a dissolving tablet; compounded versions are not FDA-approved 127.

What is vardenafil and how does it work?

Vardenafil is a phosphodiesterase type 5 inhibitor, the same class as sildenafil and tadalafil. When you are aroused, nerves in the penis release nitric oxide, which raises a messenger called cGMP that relaxes muscle and lets blood flow in. PDE5 is the enzyme that breaks cGMP down; vardenafil blocks it, so the erection you are already building holds better 1.

The label is explicit that this only happens with arousal: because sexual stimulation is required to start the release of nitric oxide, inhibiting PDE5 has no effect without it 1. No pill in this family creates desire or causes an erection while you watch the news.

How fast does vardenafil work and how long does it last?

Onset is similar to sildenafil. The label says to take it approximately 60 minutes before sexual activity, and MedlinePlus repeats the 60-minute guidance 12. Its half-life is 4 to 5 hours, and Mayo Clinic says it usually works for 4 to 5 hours 13.

The label says vardenafil can be taken with or without food, but it also reports that high-fat meals reduced peak blood levels by 18 to 50 percent 1. Mayo Clinic’s advice is to take it an hour before sex on an empty stomach, and notes that a high-fat meal or alcohol slows absorption 3. A dissolving tablet that melts on the tongue also exists; it comes in a single strength and, per Mayo Clinic, might work faster 23.

Vardenafil Sildenafil Tadalafil
Take it About 60 minutes before 1 About 1 hour before 3 About 1 hour before, or daily 3
Usual window About 4 to 5 hours 3 About 4 to 5 hours 3 Up to 36 hours 3
Half-life 4 to 5 hours 1 About 4 hours 5 17.5 hours 5
Doses 5, 10, 20 mg (2.5 mg for interactions) 1 25, 50, 100 mg 5 to 20 mg; 2.5 to 5 mg daily
Extra caution QT prolongation 1 Vision changes more common Back pain, muscle aches

What dose of vardenafil is right?

Most men start at 10 mg, moving to 20 mg for more effect or 5 mg for fewer side effects, and never taking more than one dose in a day 1. The 2.5 mg tablet exists mostly for men whose other medicines slow vardenafil’s breakdown.

Those interactions are specific. With ritonavir, the label caps vardenafil at 2.5 mg in 72 hours; with indinavir, saquinavir, atazanavir, clarithromycin, or 400 mg daily of ketoconazole or itraconazole, 2.5 mg in 24 hours; with erythromycin or 200 mg daily of ketoconazole or itraconazole, 5 mg in 24 hours 1. Men who are stable on an alpha-blocker for prostate or blood pressure should start at 5 mg 1. This is why a consult asks for your full medicine list before anything ships.

Why does vardenafil have a QT warning?

Because it measurably lengthens the QT interval, the time the heart takes to recharge between beats. In the label’s study, 10 mg lengthened the corrected QT by about 8 milliseconds and an 80 mg dose by about 10 milliseconds 1. That is small for a healthy heart and meaningful for a vulnerable one.

The label says men with congenital QT prolongation, and men taking Class IA antiarrhythmics such as quinidine and procainamide or Class III antiarrhythmics such as amiodarone and sotalol, should avoid vardenafil 1. MedlinePlus tells you to disclose any history of long QT syndrome before starting 2. If you have ever been told your QT is long, or you take a heart-rhythm drug, sildenafil or tadalafil is usually the safer pick.

What are the side effects of vardenafil?

Headache and flushing lead, as with the whole class. In the label’s trials, headache was reported by 15 percent of men on vardenafil versus 4 percent on placebo, flushing by 11 versus 1 percent, stuffy or runny nose by 9 versus 3 percent, indigestion by 4 versus 1 percent, and sinus congestion, flu-like symptoms, dizziness, nausea and a rise in the muscle enzyme creatine kinase by 2 to 3 percent each 1.

The serious warnings match sildenafil and tadalafil: an erection lasting more than 4 hours, sudden loss of vision or hearing, and a severe blood pressure drop with nitrates 12. On alcohol, the label reports that vardenafil 20 mg did not add to alcohol’s blood-pressure-lowering effect in a 4-hour study, which is reassuring but not a license 1.

Who might consider vardenafil, and how do you get it?

Men who did well on sildenafil except for the vision changes or flushing, and men who did not respond to one PDE5 inhibitor and want to try another before moving to injections. Switching within the class and adjusting the dose are standard steps in the non-responder playbook 6. In a 2013 network meta-analysis of 118 trials, vardenafil ranked second in effectiveness behind tadalafil, with no major difference in safety across the class 4.

Generic vardenafil film-coated tablets are FDA-approved, carry the same active ingredient and strengths as the original, and cost far less now that several makers compete 18. Some clinics also offer compounded vardenafil, such as lozenges or custom strengths. Compounded drugs are not FDA-approved, which means FDA does not review their safety, effectiveness or quality before they are sold 7. There can be reasons to choose one, but “same thing, cheaper” is not among them. See the vardenafil page for how we prescribe it.

When to talk to a clinician

Get emergency care for an erection lasting more than 4 hours, sudden vision or hearing loss, chest pain during sex, fainting, or a racing or irregular heartbeat 12. Never take vardenafil if you use nitroglycerin, isosorbide or any other nitrate, poppers (amyl or butyl nitrite), or riociguat; the label says no safe interval after a dose has been established, and cardiology reviews advise at least 24 hours 15.

Before starting, tell your clinician about any history of long QT syndrome or fainting, heart-rhythm medicines, alpha-blockers, blood pressure medicines, antifungals, antibiotics and HIV medicines, since each can change or rule out the dose 12.

The third rider deserves a look. The consult that comes with every plan tells you whether it belongs in your string.

Hard questions, straight answers

Is vardenafil better than sildenafil?

Not clearly. A 2013 network meta-analysis of 118 trials ranked tadalafil most effective, followed by vardenafil, with no major safety difference among the PDE5 inhibitors. Vardenafil's timing and duration are similar to sildenafil's, so most men who try it are looking for a different side-effect experience rather than more power.

How long does vardenafil take to work and how long does it last?

The label says to take it about 60 minutes before sex, and Mayo Clinic says it usually works for 4 to 5 hours, in line with its 4 to 5 hour half-life. A high-fat meal can lower the amount absorbed, so a lighter meal keeps it predictable.

Why does vardenafil have a heart-rhythm warning?

In a study, a 10 mg dose lengthened the heart's QT interval by about 8 milliseconds. The label says men with congenital QT prolongation, or who take Class IA antiarrhythmics like quinidine and procainamide or Class III drugs like amiodarone and sotalol, should avoid vardenafil.

Is compounded vardenafil the same as the generic tablet?

No. Generic vardenafil tablets are FDA-approved and reviewed for quality. Compounded vardenafil, such as lozenges or unusual strengths, is made by a compounding pharmacy and is not FDA-approved, so FDA does not review its safety, effectiveness or quality before it is sold.

Can I take vardenafil with alcohol?

In the label's study, vardenafil 20 mg did not add to the blood-pressure-lowering effect of alcohol. Even so, alcohol on its own makes erections harder and slows absorption of the medicine, so keep it modest on nights you plan to use it.

Terms and Conditions

Effective September 13, 2026. These Terms and Conditions (“Terms”) are an agreement between you and the company that operates the Hung platform (“Hung,” “we,” “us”). By creating an account, reserving a consult, or using this site, you accept these Terms. If you do not accept them, do not use Hung.

1. Who does what

Hung is a brand and a technology platform. It lets you reserve a consult, exchange secure messages, pay, and receive information. Hung is not a medical provider, a pharmacy, or an insurer, and Hung itself does not practice medicine.

Medical care is provided by LPMD Medical Group (LPMDMG), an independent medical practice, through clinicians licensed to treat patients in the state where you are located during your visit. LPMDMG clinicians alone decide whether to treat you, what to prescribe, and when to refer you elsewhere. A separate management services organization provides Hung and LPMDMG with non-clinical business and administrative support. It does not influence clinical judgment.

Medication, when prescribed, is dispensed by an independent licensed pharmacy that you choose from the options presented to you. The pharmacy is responsible for dispensing, labeling, and pharmacy counseling.

2. Who can use Hung

3. What a plan is, and is not

When you start a plan you choose a medication and a plan length and pay one price. That payment reserves a consult with an LPMDMG clinician and, if the clinician prescribes, covers the medication for the plan length. The clinician reviews your intake and health history, asks follow-up questions, and decides whether treatment is appropriate and which strength is right for you. Starting a plan is not a purchase of medication and never guarantees a prescription; the consult is a clinical evaluation whatever its outcome.

Hung is not an emergency service and does not replace primary care, urgent care, or specialist care. If you have a medical emergency, call 911 or go to the nearest emergency department.

4. Chat first, video where required

Visits are delivered by secure asynchronous messaging by default. Video is used when a state requires it for a particular service, or when your clinician decides that seeing you is necessary to treat you safely. If your visit requires video, you will be told before you pay. Response times depend on volume, complexity, and whether more information is needed. Your use of telehealth is also governed by the Telehealth Informed Consent, which is part of these Terms.

5. Fees and payment

6. Refunds

Refunds follow the Refund Policy, which is part of these Terms. In short: fully refundable before a clinician starts evaluating your case or when there is a billing error; refundable less a $45 evaluation fee when the clinician decides medication is not appropriate for you, when we determine your situation cannot safely be treated online, or when you choose not to continue after the clinician has reviewed your case; not refundable once medication has been dispensed by the pharmacy, after a no-show, or after a very late cancellation. Dispensed prescription medication cannot be returned.

7. Prescriptions, pharmacies, and shipping

8. Your account and communications

Keep your login credentials private and tell us at once if you suspect unauthorized use. You agree to receive communications about your care and your account electronically, including secure messages, email, and text messages at the number you provide. Message and data rates may apply. You can stop text messages by replying STOP; care communications may then arrive by another channel.

9. Acceptable use

Do not misrepresent your identity, location, or health; do not seek care for another person; do not attempt to obtain a prescription by deception; do not interfere with the platform, scrape it, reverse engineer it, or use it to harass clinicians or staff. We may suspend or end access for violations, for safety, or where the law requires.

10. Content and intellectual property

The Hung name, wordmark, design, and content belong to Hung or its licensors. Educational content on this site is general information and is not personal medical advice. You may use the site for your own personal, non-commercial purposes.

11. Disclaimers and limitation of liability

The platform is provided as is. To the fullest extent permitted by law, Hung disclaims warranties of any kind regarding the platform, and Hung is not liable for indirect, incidental, or consequential damages arising from your use of it. Nothing in these Terms limits the professional responsibility of LPMDMG clinicians for the care they provide, or any right you have under law that cannot be waived.

12. Disputes

If you have a problem, contact us first and we will try to resolve it. These Terms are governed by the laws of the state in which Hung’s operating company is organized, without regard to conflict-of-law rules, except where the law of your state of residence applies and cannot be waived. Complaints about clinical care may also be directed to the licensing board in your state; see State Notices.

13. Changes and contact

We may update these Terms. The effective date at the top tells you when they last changed. Continued use after a change means you accept it. Questions: support@gethung.health.
