Erection Medication With Blood Pressure Medicine: Is It Safe?

The short answer

Sildenafil, tadalafil and vardenafil can generally be combined with blood pressure medicines, and the sildenafil label found no difference in side effects between men taking it with and without antihypertensives. The two cautions are alpha blockers, where the labels say to be stable on the alpha blocker first, start the erection medicine at its lowest dose and consider separating the doses, and nitrates, which are an absolute contraindication. Expect a few extra points of blood pressure drop and watch for dizziness on standing.

Plenty of men arrive at the erection question already carrying a pill organizer. High blood pressure and trouble with erections ride the same trail, so the man who needs sildenafil often already takes something for his pressure. Fair question: can the two share a saddle?

For most combinations, yes, with a little care and a lower starting dose. Two exceptions carry the weight of this article: alpha blockers need a specific approach to dose and timing, and nitrates are never combined, full stop.

We all need a little help getting hung sometimes. Nobody should have to choose between that help and their blood pressure.

Can you take erection medication with blood pressure medicine?

Generally yes. Sildenafil’s controlled-trial safety database showed no apparent difference in adverse reactions with and without antihypertensive medication ¹. Mayo Clinic calls these medicines generally safe for men with high blood pressure who are otherwise in good health, with the loud exception of nitrates ⁵.

Care is still needed because all three erection medicines are mild vasodilators, and so are most blood pressure medicines. Stacked, they lower pressure a little more than either alone, which a well-controlled man rarely notices and a man on several agents may feel as lightheadedness on standing ¹²³.

The labels also draw hard lines at the extremes: men with resting blood pressure below 90/50 or above 170/110 were excluded from the trials, and Mayo Clinic lists very low or uncontrolled high blood pressure among the reasons to check before taking these pills ¹⁶.

How much do these medicines lower blood pressure together?

Modestly, and the labels give numbers. A single 100 mg dose of sildenafil lowered sitting blood pressure in healthy volunteers by a mean maximum of 8.3/5.3 mmHg, and added to amlodipine in men with hypertension it produced a further 8/7 mmHg reduction ¹.

Tadalafil’s studies covered more classes, from amlodipine (a further 3/2 mmHg) to angiotensin receptor blockers (8/4), enalapril (4/1), metoprolol (5/3) and the diuretic bendrofluazide (6/4) ². Vardenafil 20 mg with nifedipine added 6/5 mmHg ³.

What is the rule with alpha blockers?

Stabilize first, start low, and for vardenafil, think about the clock. Sildenafil should be started at 25 mg, and patients should be stable on alpha blocker therapy before starting it ¹. In the doxazosin studies, no man on 25 mg had a standing systolic pressure below 85 mmHg, two did on 50 mg and three did on 100 mg ¹.

Tadalafil’s label says the same: stable on the alpha blocker first, then the lowest recommended dose; for enlarged prostate symptoms, tadalafil is not recommended with alpha blockers at all ². With doxazosin 8 mg, more men on tadalafil than placebo had standing systolic pressure below 85 mmHg or drops over 30 mmHg, some with symptomatic dizziness ². MedlinePlus notes your doctor may need to change doses or monitor you ⁴.

Vardenafil adds the timing rule. Its label says a time interval between dosing should be considered with an alpha blocker, and the studies show why: given together with terazosin, one man developed symptomatic hypotension at 80/60, while separated by 6 hours, hypotension was not observed ³. Why are nitrates different?

Because the drop is not modest. The sildenafil label states that it potentiates the hypotensive effect of nitrates and is contraindicated with organic nitrates or nitrites in any form, regularly or intermittently ¹. The AUA guideline puts it plainly: the combination can cause a precipitous drop in blood pressure, so men taking nitrates should not use these medicines ⁸.

That includes nitroglycerin tablets and sprays, isosorbide in any form, and recreational poppers ¹²³. No low dose or time interval makes it acceptable; the sildenafil label says it is unknown when nitrates can be safely given after a dose ¹. Can blood pressure medicine cause erectile dysfunction?

Yes, some can. NIDDK lists blood pressure medicines and diuretics among the medications that can cause erectile dysfunction ⁷. Mayo Clinic is more specific: water pills can decrease blood flow to the penis, and older beta blockers such as propranolol are often linked with sexual problems ⁵. High blood pressure itself damages the vessel lining, so the disease and its treatment can both contribute ⁵.

Do not stop a blood pressure medicine on your own; Mayo Clinic notes that switching to a newer beta blocker improves symptoms in some men ⁵. For many men the answer is both: a regimen that interferes less, plus an erection medicine.

What should you tell the clinician?

Everything with a dose. List every blood pressure medicine, including the prostate alpha blocker you may not think of as one, any nitrate or chest pain medicine, and anything for heart rhythm, since vardenafil should be avoided with class IA or III antiarrhythmics or a congenital long QT syndrome ³. Mention lightheadedness on standing, your recent readings, alcohol, antifungals and HIV medicines, and whether the trouble began when a new blood pressure medicine did ¹²⁶⁷.

The AUA guideline treats erectile dysfunction as a risk marker for cardiovascular disease, so a good clinician wants your pressure history anyway ⁸.

When to talk to a clinician

Get emergency care for fainting, chest pain during sex, an erection lasting more than 4 hours, or sudden loss of vision or hearing ¹²³. Never take an erection medicine with nitroglycerin, isosorbide or any other nitrate, or with poppers ¹²³⁸.

Call your clinician if you feel dizzy when standing after a dose, especially if you also take an alpha blocker or more than one blood pressure medicine; sit down until it passes, and do not drive ¹²³. Uncontrolled high blood pressure, a resting pressure below 90/50, or a heart attack or stroke in the past 6 months mean sorting out the heart before the erections ¹⁶.

Can I take sildenafil if I am on blood pressure medication?

Usually yes. The sildenafil label reports no apparent difference in adverse reactions between men taking it with and without antihypertensive medication, and Mayo Clinic calls these medicines generally safe for men with high blood pressure who are otherwise healthy. The exceptions are nitrates, which are never combined, and alpha blockers, which need a lower starting dose and a stable regimen first.

How much does sildenafil lower blood pressure?

On its own, a 100 mg dose lowered sitting blood pressure by about 8/5 mmHg in healthy volunteers. Added to amlodipine, it produced a further drop of about 8/7 mmHg. Tadalafil with common blood pressure medicines added roughly 3 to 8 mmHg systolic. Those are modest numbers for most men, but they matter if your pressure already runs low or you feel dizzy on standing.

Can you take tadalafil with tamsulosin or doxazosin?

With care. The tadalafil label says men should be stable on their alpha blocker before starting tadalafil, which should begin at the lowest recommended dose. For enlarged prostate symptoms, tadalafil is not recommended in combination with alpha blockers at all. In the doxazosin studies, more men on tadalafil had standing blood pressure drop below 85 mmHg, so dizziness is the thing to watch.

Should I separate my alpha blocker and my erection pill?
